Welcome to Oregon Melanoma Center, a full service treatment center for melanoma patients. Below you will find links that will give you information about insurance, billing, registration forms, and other helpful tools.

To ensure that all relevant information is available at your initial evaluation, we request that you complete our new patient registration forms (see below “download forms”). Please plan to arrive 15 minutes early for your first appointment to allow time for registration. We will take your photo at your first appointment. If you have any questions, please call our office at 541-302-6469 or come by and see us if you are in the Eugene, Oregon-area. We request a minimum of 24 hours in advance notice for cancellation.

Your First Appointment

It’s important for you to know your insurance coverage and restrictions at the time you make your appointment. Ask if there is anything you need to do in advance of your appointment such as, obtaining a referral , prior treatment records, or imaging studies. Make an accurate list of medications, as well as any vitamins or supplements you take. Write down any symptoms you are experiencing, including any that may seem unrelated to the reason you scheduled the appointment. Bring a family member or friend along. Sometimes it is difficult to remember all the information given to you at your first appointment.
